The Sunshine Coast Health Service has a number of dental clinics and mobile dental vans providing free dental care to school children aged from 4 years up to and including year 10. When dental consent forms are issued, parents/caregivers are asked to complete the form if they wish their child to have a dental examination.
If treatment other than that of a diagnostic or preventative nature is required, another form will be issued advising of treatment needed. If you would like to proceed with treatment, you will need to sign and return the form. Parents/caregivers may choose to attend the appointment with their child.
